Medicaid Patients
(1) Referral from Doctor
All Medicaid children and adolescents must have a referral from their pediatrician. Unfortunately, we cannot accept any Medicaid child or adolescent without a referral from their doctor, indicating that it is medically necessary for them to receive outpatient counseling.
(2) Parents with Custodial Paperwork
*Please note that the entire court document/divorce decree must be sent in advance of the first appointment. Court documents must be signed by both parties, lawyers, judge, or other court official. |

*An appointment cannot be scheduled until Medicaid is aware of the primary insurance.
- Contact Medicaid and the primary insurance to update your coordination of benefits.*
- Email/fax a copy of both the primary insurance and Medicaid cards
*An appointment cannot be scheduled until Medicaid is aware of the primary insurance.
