Employee Assistance Program (EAP)

In 2018-2019, there have been many changes in insurance coverage, benefits, and eligibility for employees who have their health care through an employer or place of employment. One of the most significant changes in coverage is the use of the Employee Assistance Program (EAP).
EAP provides employees and their dependents the opportunity to receive brief, solution-focused counseling (1-12 sessions) for a duration of time (i.e., 6 months). EAP counseling requires prior authorization, which means that the employee must contact the 1-800 number to authorize these sessions.
To use your EAP benefits, you must call the EAP: (1) verify that our office is in-network and (2) activate the EAP. Once the EAP is activated, they will provide you with an authorization number. Examples of authorization numbers are: 123456789 (Cigna), 1234567 (ComPsych), and 000MATN000 (Magellan). Before scheduling an appointment, please inform us of the EAP company and authorization number in order for us to prepare all the paperwork.

EAP Paperwork
Clients who plan to use their employee benefits (EAP) may have additional forms to complete, depending upon which EAP their employer/company carries. There are many different EAP companies, so it will be beneficial for you to find out which EAP company your employer carries prior to your first appointment.
For your convenience, the additional paperwork for these EAP companies are listed below for download.
