Clients with Insurance Policies

- Contact your insurance to verify eligibility and benefits.
- Does your plan cover behavioral health/mental/nervous?
- Does the deductible apply?
- If so, how much has been applied towards the deductible?
- Is there a copay (office visit or specialist)?
- Does your plan cover individual, family, or couple's counseling?
- Is prior authorization for counseling required?
**Please note that while you have health insurance (BCBS, Cigna, etc), it does not necessarily mean that you have behavioral health or mental health coverage. To prevent future billing problems, it is strongly recommended that you contact your insurance company to learn whether outpatient psychotherapy is a covered benefit under your plan.
Clients with Medicaid
Parents with Custodial Paperwork
*Please note that the entire court document/divorce decree must be sent in advance of the first appointment. Court documents must be signed by both parties, lawyers, judge, or other court official. |
